Immunizations for FUSD Students with Medi-Cal or Uninsured Only
Only (1) Parent/Legal guardian (Present caregiver affidavit from FUSD Project Access) can accompany the student.
Parent/Legal guardian and student must wear a properly fitting mask at all times over nose and mouth
Child cannot be given immunizations without a parent or legal guardian. No siblings, aunts, uncles, grandparents, girlfriend, boyfriends or step-parents (unless they are legal guardians) can give consent
MUST bring current immunization record
MUST bring current Medi-cal card
*If your student has private insurance please call their doctor or call any pharmacy that administers immunizations; i.e. CVS, Rite Aid, Target, Costco – to see if they have the immunizations your child needs, accept your insurance, times etc.
Our FUSD Mobile Health Unit and Tioga Clinic is open by appointment only.
